Please check out this blog entry from my friend Alan at Thirty-nine and Holding.  He needs a service that is supposed to be covered by his insurance (Kaiser Permanente) but they seem to have a history of refusal to cover the service.  Like many insurance companies the all mighty dollar is more important to them the health and wellbeing of human being.
